View, manage, and complete your active forms on the web platform

The Your Forms tab gives you a personal workspace to view, manage, and complete all of your own active forms in one place.


Viewing and Completing Forms

  1. Select the Your Forms tab.

  2. Double-click any form to open it and pick up exactly where you left off.

  3. Edit fields directly in the browser as needed.

  4. When ready, click Submit to complete the form.

  5. Once submitted, the form moves automatically to the Submissions tab.


Starting a New Form

  1. From the Your Forms tab, click Start Form.

  2. Select a template from your library.

  3. Complete the form and submit when ready.


Managing Your Forms

With the right permissions, you can take the following actions using the three-dot menu on any form:

  • Reassign β€” Pass the form to another team member. Select Reassign, choose the new assignee, and confirm. The form will appear in their In Progress list on the mobile app, or in their Your Forms tab if they are working on web.

  • Duplicate β€” Copy the form with all fields intact, ready to continue working on. The original is preserved.

  • Activity Log β€” View a full history of changes, including what was updated, when, and by whom. Compare two versions side-by-side to see exactly what changed.


Use Case

This feature is for any Chime user who needs to complete and manage forms on the web platform. It is particularly useful for those who prefer working on a larger screen, or who need to review, edit, and submit forms with more detail than is practical on mobile.

  • Double-clicking a form takes you straight back to where you left off β€” no need to start from scratch.

  • Use Duplicate if you need to complete a similar form more than once, saving time on re-entering repeated information.

  • Check the Activity Log to keep track of any changes made to a form, useful for audit trails and quality assurance.
