On Chime, you can build forms that include scored questions, with a pass/fail threshold. This gives you deeper insight into performance and compliance β ideal for inspections, audits and assessments.
Scoring works with specific widgets only:
Yes/No (all variations)
Red/Amber/Green (all variations)
Multiple Choice (Single Answer only)
Start building your form
Go to the Forms tile and click the Checklist tab.
Click Add New Checklist.
Give your checklist a name and tag.
Expand the Additional Options section.
Toggle Enable Scores to green β β this activates scoring.
(Optional) Toggle Show Section Scores if you want users to see scores per section on the PDF.
(Optional) Under Notifications, choose who should be alerted if the form fails its scoring criteria.
Example: Red / Amber / Green
Press Add Section, click the pen icon, and set:
Title: eg. Welfare Check
Type in Pass Score (this is the total for the whole section): 0
Press OK
Add your first field:
Choose Widget: eg. Red / Amber / Green
Write your Label: Are the toilets clean?
Score settings:
π’ Green = 0 (pass)
π Amber = -1
π΄ Red = -1
NB: You can set a Minimum Threshold (Hold Point or Hard Stop) β this will mean that the person cannot continue until the next question unless the minimum threshold has been met.
Click OK
Multiple Choice Example
Add a new section:
Give it a title: eg. Office Block
Pass Score: 20
Press OK
Add a field using Multiple Choice (Single Answer only):
Write your Label (question): Which block is the biggest?
Choices:
Block A β 0 points
Block B β β 20 points (correct)
Block C β 0 points
Block D β 0 points
Click OK
When all sections are built, press Save & Close.
Once the checklist is carried out, youβll be able to see total scores, section scores, and whether a form passed or failed.

This feature is mainly used by Admins, Super Users, Health & Safety Managers, or anyone designing forms that need to track results. It helps standardise what βsuccessβ looks like across different checks and automatically alerts the right people when something fails.
