Embed video into a form / checklist

Chime’s Forms allows you to embed video content directly into your checklists, giving you a flexible way to share visual guidance - whether it’s an site induction video, training or toolbox talk.

To embed a video, it must first be hosted on YouTube or another platform that provides a valid video URL. Once linked, the video plays directly within the form, and you can even require users to watch the full video before submitting.


Adding video to your Form

  1. Select the Forms tile on the left-hand side

  2. Open an existing checklist or create a new one

  3. Add a new field to your form and choose the Video widget

  4. Give your field a clear label (e.g. Site Induction Video)

  5. Toggle Required if you want to make it mandatory for users to watch the full video

    • If this is on, users cannot skip or fast-forward the video and must watch to the end before submitting

  6. Paste your video URL (must be a valid YouTube or equivalent link) into the video link field

  7. Press OK to add the video to your form

  8. Continue building your form — you can follow up the video with related questions if needed

  9. When finished, press Save & Close\


Use Case

This will typically be used by Admins, Super Users, or Health & Safety Managers — anyone building forms to be completed by the wider team.


💡 Tips

  • Use videos for inductions, overviews, or safety briefings

  • Always test the video link to ensure it loads correctly

  • Make it "Required" when you want full view-through before submission

  • Pair video content with follow-up questions to confirm understanding
