When building forms in Chime, widgets give you the flexibility to capture exactly the information you need. From simple text fields to photos, videos, signatures, and drop-down lists, widgets make forms dynamic, user-friendly, and easy to complete. They also ensure accuracy and consistency by guiding users with the right input type for each situation

Widget Options Available

Text Options

  • Instructional Text – Add guidance notes to help users complete the form.

  • Single Line Text – For short, one-line responses.

  • Multi Line Text – For longer, free-text entries.

Number Options

  • Number Text Box – Capture numeric values only.

Date & Time Options

  • Date & Time – Select from a calendar and time scroll.

  • Date Only – Choose a date from the calendar.

Signature Option

  • Signature – Sign with a finger on screen and include a name.

Red / Amber / Green Options

  • RAG Choice – Options include:

  • Red / Amber / Green

  • Red / Amber / Green with Comment

  • Red / Amber / Green / Not Applicable

Yes / No Options

Flexible variations including:

  • Yes / No

  • Yes / No with Comment

  • Yes / No / Not Applicable

  • Yes / No with Photo

  • Yes / No / NA with Photo

Multiple Choice Options

  • Create different options and set how many answers are allowed.

Image & File Upload

  • File Upload – Attach files directly.

  • Display Image – Upload an image for users to view.

Photo Options

  • Single Photo, Multiple Photos, or Multiple Photos (Captioned).

  • Restrict Camera Role - you can restrict users from selecting from their camera roll so they can only take 'live' photos

  • GPS Location - you can get a GPS location, time & date stamp for each photo taken

Video Option

  • Add a video URL (e.g. YouTube) or embed Luma training courses.

Link the form to a specific:

  • Site

  • Employee

  • Client

  • Supplier

  • Equipment/Asset

Formatting Options

  • Page Break – Insert sections into your form.

  • Display PDF – Upload a PDF to be shown within the form.

Time & Money Requests

  • Create Time Request – Add dropdowns of time request options.

  • Money Request – Enter amounts and VAT for payment.

Site Tasks

  • Embed site tasks within a form, e.g. another checklist.


💡 Tips

  • Use Instructional Text to keep forms clear without cluttering labels.

  • Combine Yes/No with Photo to capture both confirmation and visual evidence.

  • Use Regarding Options to tie form responses back to key records (sites, employees, or equipment).

  • For compliance, always include a Signature widget where acknowledgement is required.
